Persistent Jaw Pain and Cavitation – When a Past Extraction Still Hurts

Presenting Complaint:

The patient presented with chronic pain in the lower right jaw, including tenderness, discomfort radiating toward the ear, and pain in the temporomandibular joint (TMJ) area. The symptoms have been ongoing since a tooth extraction, raising concerns about a jawbone infection after tooth extraction or a silent tooth infection not visible through regular dental exams.

Advanced CBCT Imaging – Region LR6

A Cone Beam CT (CBCT) scan was carried out to explore possible causes of dental infection with no symptoms. This scan provides a 3D view of the jawbone and is a preferred tool for identifying issues that traditional X-rays may miss—particularly when dealing with a cavitation in jaw bone.

Advanced CBCT Imaging

Findings:

  • A 3-dimensional radiolucent area around site 45, measuring approx. 5mm × 12mm.
  • A radiolucent channel extending toward the previous extraction site, suggesting jawbone not healing after tooth extraction.
  • Communication with the oral cavity, pointing toward a residual infection or toxic jawbone lesion.

These are classic signs of a dental cavitation, a hidden lesion where the bone hasn’t healed fully. Patients often describe symptoms like lingering pain, fatigue, or unexplained discomfort long after a dental procedure terms they associate with:

  • “Dental cavitation symptoms”
  • “Toxins in jaw bone”
  • “Fatigue and jawbone infection link”

Diagnosis and Differential Considerations:

  • Chronic localized osteitis / cavitation (neuralgia-inducing cavitational osteonecrosis – NICO)
  • Residual infection from incomplete extraction
  • Low-grade chronic osteomyelitis
    These conditions are increasingly searched under terms like:
  • “Cavitational osteonecrosis treatment”
  • “Osteonecrosis of the jaw”
  • “Biological dentist for jawbone issues”

Additional Findings – Tooth UR6

The CBCT scan also revealed a periapical infection involving the mesiobuccal, distobuccal, and palatal roots of tooth 16 (UR6). This has extended into the maxillary sinus, evidenced by thickening of the sinus floor lining, a common sign of odontogenic maxillary sinusitis.

Diagnosis:

  1. Chronic apical periodontitis (Tooth 16)
  2. Odontogenic sinusitis

Treatment Plan

Region LR6 – Suspected Cavitation Site:

A surgical exploratory procedure was advised to treat the suspected jawbone infection, commonly referred to by patients as:

  • “Holistic treatment for dental cavitations”
  • “Toxic jawbone infection symptoms”
  • “CBCT scan for dental infection”

Procedures Included:

  1. Explorative Surgical Access:
    • Full-thickness flap raised.
    • Curettage of necrotic bone using Lucas curettes.
    • Irrigation and removal of infected tissue.
  2. Biological Healing Support:
    • Ozonated water (45 γ) applied to the site.
    • Placement of ozonated gauze for 30 minutes.
    • Photobiomodulation laser therapy (SIROLaser, 660 nm, 25mW) administered before and after the procedure to promote healing.
  3. Management of the Bony Defect:
    • Consideration of bone graft or antimicrobial placement depending on the size and nature of the lesion.
  4. Postoperative Monitoring:
    • Follow-up includes symptom tracking and potential re-imaging.
    • Further testing may be required if infection persists.

Tooth UR6 (Upper Right Molar):

  • A biological extraction of tooth 16 was performed to remove the source of infection.
  • Healing will be supported with antimicrobial and regenerative therapies.
